C.N.A

Conduct resident rounds as assigned. Participate in the development of an individualized plan of care of assigned residents.  Document in the nursing assistant notes the care and treatment provided to the resident.  Maintain confidentiality.

Assure residents have call lights at hand and answer call lights promptly.  Assure residents are given adequate assistance with meals, nourishment and transportation to meal service areas.  Assist residents with activities of daily living as documented in the resident’s care plan.  Work as a team player, demonstrate dependability, and present an appropriate professional appearance.

Med Aide

Successfully complete the 40-Hour Medication Aide Course approved by the State of Nebraska.  Obtain report from nurse being relieved and follow through with recorded medication changes.  Administer medications to residents in an accurate, timely manner.  Document medication administration, related monitoring, effectiveness and any occurrence of adverse effects.  Complete all documentation related to the resident’s medication administration within scope of practice.

Registered Nurse

Interpret existing policies and procedures to licensed nurses, nursing assistances, restorative nursing, residents, families and physicians.  Supervise and evaluate all direct resident care and initiate corrective action as necessary.  Conduct resident rounds daily and initiate corrective action.  Identify resident problems and emergency situation and initiate. 

Follow through with mandated Resident’s Advanced Directives. Administer and document direct resident care, medications and treatments per physicians’ orders and accurately record all care provided.  Promptly report changes in residents’ conditions to the physician, Director of Nursing and responsible party, and take follow-up action as necessary.

Maintain confidentiality.  Work as a team player, demonstrate dependability, and present an appropriate professional appearance.

Certified dietary manager

To implement and maintain effective, efficient systems to operate the dietary department and provide food service to residents in a cost-effective, efficient manner to safely meet residents’ needs in compliance with federal, state and local requirements

Qualifications:

  1. Meet at least one of the State of Nebraska qualifications of Dietary Manager, and participate in at least 15 hours of continuing education related to Dietetics each year.
  2. Have experience with and understanding of the needs of the elderly, including care plans and Medicare/Medicaid requirements and both skilled and assisted living units.
  3. Have knowledge of menu planning and writing.
  4. Have knowledge in sanitation, Board of Health regulations, special elderly dietary needs, and food handling.
  5. Qualify for certification by the Dietary Manager’s Association.
  6. Must have excellent oral and written skills
  7. Possess personal qualities that enable the employee to maintain good interpersonal relationships with employees and residents within the facility.

Job Duties:

Must be flexible to work hours needed and work at either facility, follow directions. Plan diets and menus for review by the consultant. Purchase or requisition food and supplies. Supervise the receiving and storage of food, food preparation and service. Complete assigned MDS in a timely manner. Review residents’ dietary profile, assessment plan and plan of care for residents triggering a Q1 related to nutritional status and eating function to determine if there is a problem or concern that can be resolved, improved or addressed in a different manner to improve the resident’s nutritional status and eating function. Interview residents and determine specific dietary needs and preferences. Participate in employee selection, evaluation and termination. Be responsible for communicating pertinent information to dietary employees.

L.P.N

The purpose of this position is to supervise, provide and coordinate nursing care in compliance with facility policies and procedures and to assess residents’ care needs and take appropriate action and to report resident’s needs and concerns to Director of Nursing.

Interpret existing policies and procedures to nursing assistants, restorative nursing assistants, residents, families and physicians. Obtain and provide report from nurse being relieved and record sufficient information to implement appropriate follow-up action as necessary.

Provide instruction for appropriate nursing care to licensed nurses and Nursing Assistants with in scope of practice.
Identify resident problems and emergency situation and initiate. Follow through with mandated Resident’s Advanced Directives.
